It is important to bring your current insurance card(s) to each visit and inform us of any changes in your coverage or to your home address. As changes may occur in the plans that we accept, please verify our acceptance of your plan at the time you make an appointment.
Benefits and Eligibility
Although Northern Illinois Retina will bill your insurance, full payment ultimately is your responsibility. We will make every effort to verify your coverage prior to your appointment, but we cannot guarantee coverage of services by your insurance company. To determine your insurance benefits and eligibility, please contact your insurance carrier directly. Please note that you are responsible for payment of all services that are not covered by your insurance plan. Any patient balance after insurance is normally due within 30 days, but we can set up a payment plan for outstanding accounts. We also request payment or co-payment for office services at the time they are rendered unless prior arrangements have been made.
If you belong to an HMO or PPO, please call us in advance. This will ensure that you have the necessary forms and authorization. Without them, your HMO or PPO may not allow you to see our doctor. Please free to call us about any financial arrangements.
